Systematic review of neonatal seizure management strategies provides guidance on antiepileptic treatment

Abstract

There is a lack of scientific evidence to support the best management of neonatal seizures. Current strategies for neonatal seizure management were investigated by analysis of all surveys published during the time period 2000–2012. Methods for seizure diagnosis and availability of electroencephalogram (EEG), including monitoring, varied. Phenobarbital was the drug of first choice, and the use of off-label drugs and treatment times varied. We conclude that there is an urgent need for more evidence-based studies to guide neonatal seizure management.

Minimal enteral nutrition during neonatal hypothermia treatment forperinatal hypoxicischaemic encephalopathy is safe and feasible

Abstract

Aim: The safety and efficacy of enteral feeding during hypothermia treatment following hypoxic-ischaemic encephalopathy has not been studied before, resulting in variations in practice. Our study compared the benefits and safety of both early minimal and delayed enteral feeding during hypothermia treatment.

Methods: Our retrospective cohort study, from January 2009 to December 2011, compared a Swedish cohort, who received early enteral feeding during hypothermia, and a UK cohort, who received delayed enteral feeding.

Results: In Sweden (n=51), enteral feeds were initiated at a median of 23.6 h and full oral feeding was achieved at 9 days (range 3–23). In the UK (n=34), the equivalent figures were 100 h and 8 days (range 3–13) (p=0.01). Both groups achieved enteral feeding at a median 6 days. The median length of hospital stay was 13 days in Sweden and 10 days in the UK (p=0.04). More babies were fully breastfeeding or breastfed and bottlefed at discharge in Sweden (85 %) than the UK (67 %) (p=0.08). There were no significant differences between the two groups regarding adverse events.

Conclusion: Early minimal enteral feeding during hypothermia proved feasible, with no significant complications. Delayed enteral feeding did not affect time to full enteral feeding.

Brain-Lung-Thyroid syndrome: genetics, pathogenesis, clinical presentation, therapy

Abstract

Brain-Lung-Thyroid syndrome is a rare genetic disease characterized by pathology of the same organs. Authors present data about genetics, pathogenesis, clinical presentation, therapy of this syndrome. Analysis of literature reports about 50 patients with this disease from 1998 to 2014 years, and allowed to reveal its phenotypic variability. Authors represent their own clinical case of patient with genetically confirmed BLT syndrome.

The effectiveness of parenteral glutamine supplementation in critically ill premature infants

Abstract

The overview of the recent studies of parenteral glutamine supplementation in critically ill preterm infants was presented. The proposal mechanisms of positive effects and pathogenetic explanation of the glutamine administration to prevention of dangerous and frequent intensive care complications in very low birth weight neonates. The results of meta-analyses and systematic reviews were discussed. Despite to safety, the results are controversial, partly because of methodological errors and limitations in some studies. It is necessary to performe well-designed randomized controlled studies of parenteral glutamine administration in a population of premature infants as a perspective direction of research, the significance of glutamine in neonatology.

Parameters of physical development of children given rise before term in the Voronezh Region

Abstract

Aim – to study the parameters of physical development of the children born in gestation from 22 to 36 weeks in the Voronezh region and make a table sigmalny chart type and physical development of premature children.

Results. 1 595 stories of development of newborns (the girl – 797, boys – 798) with gestational age were studied at the birth from 22 to 36 weeks for 2009–2013. On their basis tables of sigmalny type and the chart of physical development of prematurely born children depending on gestation term are made at the birth, a floor, masses of a body, are long bodies, circles of the head, a circle of a thorax, Ketle I.

Conclusions. The received results are of great importance for an assessment of a condition of the newborn at the birth, reflect features of its pre-natal development and his lives are predictively significant the next years.
